How Do I Connect My Medical Records to HealthTree Cure Hub?

Connecting your records with HealthTree Cure Hub has never been easier!

There are three main ways to connect your records—the type of connection will depend on the system your facility uses and whether or not you have access to a patient portal:

1. Automatic Connection: If you have a patient portal and your facility uses EPIC, Cerner, or VA Hospital systems, you can automatically link your patient portal to your HealthTree Cure Hub account, and your information will be synchronized.

2. Electronic Connection: If you have a patient portal but your facility doesn't use the above-mentioned systems, you can provide your credentials (username and password), and our team will manually add the information for you.

3. Paper records: If you don’t have a patient portal, you can still connect your medical information to HealthTree Cure Hub by giving us consent to request your paper records directly from your facility. Our team will manually add the information for you.

Note: If you're not interested in any of the options mentioned above or you are

receiving treatment outside of the United States and want to connect your

medical records, please email us at [email protected].

Each of these options is explained in detail below—keep reading to learn how to connect your records.


Automatic Connection

1. Go to the “Connect Medical Records” section on your Cure Hub profile

2. Click on “Add new connection.”

3. Look for your medical facility in the drop-down list and select it. (If you

don't see your facility in the drop-down menu, try with a different alias;

if you still don’t find it but you do have a Patient Portal, follow the

instructions under the Electronic Connection section.)

4. It will ask if you have a Patient Portal. Select "Yes".

5. You will be redirected to your Patient Portal website. Log into your portal

as you usually do. (Your username and password will not be saved on

our system; it's only requested to help create the connection).

6. Select the information you’d like to share and how long you’d like the

connection to remain in effect. (We recommend selecting the longest available duration)

7. Once the connection is established, your medical records will automatically migrate to your HealthTree Cure Hub profile. Please note that test results that cannot be directly mapped, such as imaging studies or genetic reports, may take up to six weeks to be incorporated into your profile. This is because they require thorough review and manual entry by our Medical Navigators.

8. Every time you have new information, you can update it on your HealthTree Cure Hub profile by clicking on “Sync Now.”

9. Repeat this process for any other facilities you would like to connect.

10. You can delete or remove any connections at any time.

Electronic Connection

  1. Go to the “Connect Medical Records” section on your Cure Hub profile.

  2. Click on “Add New Connection”

  3. Look for your medical facility in the drop-down list and select it (If you don't find your facility in the drop-down menu, please, click on "Tap here to add facility" and enter your facility info).

  4. It will ask if you have a Patient Portal. Select "Yes".

  5. Add your portal username and password, sign digitally, and check the box that you authorize the HealthTree Foundation to request your medical records and complete your HealthTree Cure Hub profile.

  6. Click on the "Connect" button at the bottom.

Paper records

  1. Go to the “Connect Medical Records” section on your Cure Hub profile.

  2. Click on “Add New Connection”

  3. Look for your medical facility in the drop-down list and select it (If you don't find your facility in the drop-down menu, please, click on "Tap here to add facility" and enter your facility info).

  4. It will ask if you have a Patient Portal. Select "No".

  5. Add your digital signature and check the box authorizing the HealthTree Foundation to request your medical records and complete your HealthTree Cure Hub profile.

  6. Click on the "Connect" button at the bottom.
