How can I connect my medical records with Cure Hub?

Connecting your records with Cure Hub is easier than ever! There are four main ways to connect your records; choose which option is best suited for you. If you have a patient portal and your facility uses EPIC, Cerner, and VA Hospital systems, then you can connect your facility to your Cure Hub account. All your health information will automatically download to your Cure Hub account. If your facility doesn't use these systems, but you still have a patient portal, then you can choose to give access to us to use your patient portal and manually enter the information into your Cure Hub account. If you don't have a patient portal, then you can give access to paper records. Lastly, you can send your records directly to us if you don't have a patient portal and are being seen at a facility outside of the United States.

Each of these options is laid out below; read more below to connect your records.


You have a patient portal, and your facility uses EPIC, Cerner, or VA Hospital systems.

We have been working hard to make major improvements to the Cure Hub platform. Now you can connect Cure Hub electronically to all EPIC, Cerner, and VA Hospital systems. This means no more manually entering data into your profile! If your facility uses these platforms and you have a patient portal, then you can connect your records by following the steps below:

1. Go to the “Connect My Records” section on your Cure Hub profile

2. Click on “Add new connection.”

3. Look for the name of your medical facility and click that you do have a patient portal

*if you don't find your facility in the drop down menu please let us know*

4. The portal for your medical facility will open up-> log into your portal by using your username and password (this information won't save with us; it’s only requested to help you create the connection)

Once your records are connected, any labs or other tests will automatically appear in your Cure Hub profile. You can choose how long you want the connection to remain in effect (we recommend selecting the longest available option to keep your profile updated), and you can delete or remove any connections anytime you wish.


You have a PATIENT PORTAL, but your facility is not connected with the above systems.

No worries if your facility does not use this system, you can still add your records to your Cure Hub account, and our team will manually add the information for you.

To add your records to your Cure Hub account with your patient portal, follow the steps below:

  1. Go to the “Connect My Records” section on your Cure Hub profile.

  2. Click on “Add New Connection”

  3. Add the name of your facility.

    *If you don't find your facility in the drop down menu plase, click on "Tap here to add facility" and enter your facility info*

  4. Select that you do have a patient portal with the facility and select the "Next" button.

  5. Add your portal username and password, sign digitally, and check the box that you authorize the HealthTree Foundation to request your medical records and complete your HealthTree Cure Hub profile.

  6. Click on the "Connect" button at the bottom.


You want to give consent for HealthTree to request PAPER RECORDS.

If you do not have a patient portal, you can still add your records to HealthTree by giving us consent for paper records. This allows us to send the consent form to your facility and have them send us back your records. Once we receive your records, we upload them to your Cure Hub account and email you, letting you know you can view your lab values and genetic testing on your account.

To give consent for paper records, follow the steps below:

  1. Go to your Cure Hub account and click on the section Connect My Records.

  2. Click on the green + button or the Add New Connection button.

  3. Type the name of your facility.

  4. Click on the button "No" stating that you don't have a patient portal, and select "Next."

  5. Add your digital signature, check the box authorizing the HealthTree Foundation to request your medical records, and complete my HealthTree Cure Hub profile. Then select the green "Next" button.

That is all needed to add your consent to request your paper records from your facility. We will send this consent form to your facility and will email you when we have added all your records to your Cure Hub account.


You want to send your records to the HealthTree Foundation manually.

If you are not interested in any of the above options to add your records to your Cure Hub account or are getting treatment outside of the United States, then the best way to add your records to your account would be to send them directly to us. You send us your records directly by sending an email to [email protected] and attach the records to the email. In the email, let us know you want them added to your CureH Hub account, and we will add them.
