A patient portal is an online platform that allows you to access and manage your personal health information. Through the portal, you can:

  • View your medical records, including your medical history, lab results, medications, allergies, immunization records, and more.

  • Schedule appointments and refill prescriptions.

  • Communicate with your healthcare providers.

  • Track upcoming appointments and current medications.

Patient portals are often branded differently depending on the provider. Some common names include MyChart, MyCarePlus, Navigating Care, and CareSpace.

Connecting Your Patient Portal to Cure Hub

You can connect your patient portal to your HealthTree Cure Hub account to help you better manage your disease. By linking your portal, you can:

  • Track your lab results and medical data.

  • Discover personalized treatment options and clinical trials.

  • Participate in research initiatives.

  • Find and connect with your genetic twin matches.

  • Schedule calls with our Medical Navigators, who can help you understand your lab results and prepare for doctor visits.
